Time’s Up: Revolutionizing Our Dental Care Crisis Now!


Picture this: Your knee is screaming in pain, every step feels like a battle, and you find yourself among the 1.3 million Americans who need a knee replacement each year. But the nightmare doesn’t end there – your knee is not just worn out; it’s infected and so damaged that walking has become a Herculean task. After a thorough consultation, complete with all your questions answered, you make your way to the front desk to schedule your surgery, and then the bombshell drops. The price? More than $30,000 for a single knee.

“Don’t worry!” the patient coordinator chirps with faux cheerfulness. “Your insurance will cover a whopping $2,000, and we have financing options! You could even consider remortgaging your home!”

Stunned, you inquire about alternatives, and you’re met with a chilling suggestion: “Well, we could just remove the knee altogether and give you these acrylic legs. Sure, you won’t be able to run or feel anything, and it will take months to adjust, but hey, it’s better than suffering, right?”

Can you imagine the uproar from politicians if such a scenario unfolded in a medical office? Yet, this reality plays out every single day in dental clinics across the nation, often without a peep of protest.

Dental care is not a luxury; it’s a vital component of our overall health and well-being. Yet, for countless residents in Maine, access to quality dental care remains a distant dream due to sky-high costs, a shortage of providers, and pathetic insurance coverage. This is not just an individual crisis – it has far-reaching consequences for our healthcare system and economy. It’s high time for lawmakers, insurers, and the dental profession to step up, tackle the lack of coverage for dental treatments, and ensure everyone has equal access.

The biggest obstacle to affordable dental care lies in the archaic reimbursement structure of insurance plans. The annual cap of $2,000 has remained stagnant for decades, even as the costs of providing care have skyrocketed. Although some legislation is in the works to force insurance companies to give back some profits to patients, what we truly need is a radical overhaul of the system. Recent studies show that nearly one-third of Americans skip routine dental visits simply because they can’t afford them.

In Maine, where median incomes often lag behind the national average, the impact is even more severe. Families struggling to make ends meet, seniors on fixed incomes, and even middle-class residents are left with tough choices: forgo necessary dental work or sacrifice basic needs like housing and groceries. Insurers must be mandated to cover 100% of medically necessary procedures. There’s no other way to prevent the grim decision between paying rent and losing a limb.

In a previous piece, I advocated for the establishment of dental therapy programs, even offering my own Auburn dental office as a training ground for aspiring dental therapists. Despite my outreach to college deans, educators, and dentists who successfully launched similar programs elsewhere, my proposals fell on deaf ears, met with indifference from those who wield the power to make it happen.

Despite months of unanswered emails and pleas, I found myself in a frustrating state of powerlessness as I tried to advocate for meaningful change.

However, solutions to this crisis do exist, but they demand bold and decisive action. Expanding Medicaid coverage to include full mouth replacements is an essential first step. Maine’s recent move to offer dental benefits to adult Medicaid recipients is a welcomed development, but it’s merely a drop in the bucket. We must treat dental needs with the same seriousness as medical needs, and Maine is in a unique position to pioneer a solution to this crisis. Senator Susan Collins has pledged to support Mainers in her new role as Chair of the Senate Appropriations Committee, and I believe she could enact substantial change if she understood the immense challenges our fellow Mainers face.

By prioritizing dental therapy and boosting insurance reimbursements, we can uplift the lives of countless residents and fortify the health of our communities. The time for action is now!
